A few tips that are scarcely mentioned (you'll have plenty in the links cited by David Brown)
Don't overindex
Index with synonyms (eg font, character, size, courier, arial, bold, etc.)
try to check that the indexing covers all the book (you can sort the index by page numbers, put it in excel for instance, then sort the page number column)
A two level index (Size: character, circle, etc) is more difficult to build, but it allows you to easily emphasize the definition, or glossary entry
There are hoards of resources about indexing, so that'll be all. Which is not true: having a critical look at bad indexes is a good way to learn what should be avoided. Plus note that a poor index is worse than none (I thin of a two page lame thing at the end of a 500 page books. This is a glossary to be, not an index) Ans also, sometimes a glossary is better than an index.
