SAP 4.6: customizing context-sensitive plainHTML Help

Does anyone have any experience in customizing "plainHTML Help" for SAP
4.6?

Our SAP applications require a great deal of custom context-sensitive
online Help. In 3.0D we did this using THEXC tables in SAP linking specific
transactions to particular WinHelp topics which we custom built in
RoboHelp.

In 4.6, we would like to integrate these customized Help topics in the
plainHTML Application Help provided by SAP. It is easy enough to use any
HTML editor to modify particular content frames, but how do you edit other
aspects of the Application Help, namely:
*Add Glossary Items?
*Customize the TOC frame?
*Add new topics and link them to the TOC and Search features?

If SAP's Application Help is not customizable--or requires a lot of deep
coding--please offer your suggested solutions for custom context-sensitive
online Help, preferably Intranet deployed.
