i came in at the middle of this thread so i don't know if anyone has mentioned CCRider from westernware ( www.cc-rider.com) or Imagix (www.Imagix.com), both are excellent tools, very powerful and provide a variety of outputs including tree views, HTML, RTF.
If you are looking for document-generating tools only, I've used DocuClass. It's an inexpensive little tool, $50 US, and does a decent job with classes, members, functions, etc., but does NOT handle namespaces.
