I've had a few requests to put something up to show that Acrobat forms can,
indeed, be used to load information into an online database. I spent a few
hours this weekend putting something together.

I have included a link to a page that shows the source for each of the JSP
pages, in case you are a closet code hacker, and might be able to pick up on
how it works. :-)
