Yes, you can. At least in Acrobat 4.05. You can do it manually through
Acrobat or automatically through pdfmark or a special plug-in (I'd have to
find out where that is).

Yes and yes.

My first page is called "Title." My second page is "Legal." My preface and
TOC are lowercase Roman numerals. The main text of my doc uses Arabic
numerals starting with page 1.

I'm pretty sure you can't print the bookmarks without doing an old-fashioned
screen shot.


Something else we do is format our online PDF pages to be smaller than our
print-ready PDFs. That way readers can view the entire page without having
to scroll, much like they would a good HTML page. We also use many hypertext
links that make navigating the PDF easy.
