Easy answer: I don't create PDFs from HTML pages (unless archiving a site).
:)

I maintain my source in a document (DOC or FM, usually), set it up for
print, and print to PS/distill to PDF. Then I repurpose the document to
HTML.

Creating a PDF of an HTML document defeats the purpose of supplying a
"print-friendly" PDF, as your PDF will contain the same "errors" as regular
ol' printed HTML pages.
