Just yesterday. Running through the product (web-based RF tracking tool) one
last time and checking it against the "final draft" of the docs. Noticed a
new link on the MAIN PAGE! Checked it out. Much to my
amazement/disgruntlement, there's an entirely new tool/process for doing
something completely outside the development specs of the tool.

So I was off to ask the project manager about it.

"Oh that. Well, a few of the field guys wanted the be able to..."
"So I told development..."
"Hmmm? No, I .. um... guess I just forgot to tell you. "

Arrrgh!

That's got to be number one for me.
