We create our HTML documents in HomeSite, including TOC pages.

We use HTML Indexer to index them, naturally. :) It works with any
HTML source files, no matter what tool you use to create them.

If you already own Dreamweaver, take a look at Ben Weisner's products,
advertised at the bottom of every Techwr-L message:
