I should have added that when you create your thumb index boxes, you should extend them one-eighth inch past the edge of the finished trimmed page. This is the standard bleed margin and ensures that the cut will go through the box rather than just miss the edge of it, as the blade descends through a stack of printed pages. By the same token, you want to keep any "live copy" (that is, type) at least one-eighth inch in from the finished trim. You should discuss this with the printer or bindery, though, as they may require a large margin.
