You could try the Mr. T of outliners - FrameMaker+SGML. I pity the foo that uses <insert maligned product name here>. There's no limit to what FM+SGML outlining can do. (Only half-joking... Ok, 3/4 - costs more than freeware, and you have to set it up. But if you already have it, it will change your definition of an outliner... I use it all the time.)
