You never got an answer to this question, did you?

It's because the page number in a Word index entry isn't hyperlinked,
whereas the page number in a Word TOC *is*. Word is just reproducing
that in the HTML.

(Don't ask me why Word's index entries aren't hyperlinked in the first
place, though.)
