The missing links in the PDF file turned out to be a corrupted Word document.

To get it working, I copied the contents of the original file into a new document and ran it through the distiller. Acrobat converted the new version a lot faster and created the links. The Word file had been through many hands and many revisions so there's no telling what kind of garbage was in there. I can tell you how much though - the new version was a meg smaller than the original.
