For me, one of the most interesting aspects of this discussion (which I have not read in its entirety yet) is this: I think of the index and other retrieval mechanisms as part of the CONTENT, not the design. Therefore if you can't find information that's actually there, it's a content problem, not a design problem.

Maybe the index, partitioning into sections, etc. can usefully be viewed as a third category separate from both content (in a narrower sense) and design (in a narrower sense): structure.
