Some examples from my experience:

Being asked to install software on my PC that was
installed elsewhere and for which there was only one
seat, because the boss saw no reason to spend his
budget on additional copies.

Being in a position to see somebody with seniority
force decisions on documentation that were bad for the
documentation and hence the product--no, cost was not
an issue here.

Seeing a manager tell a bald-faced lie that was
injurious to an employee lower in the food chain.

Noticing an employer misrepresenting themselves to a
candidate at an interview.

Witnessing an employer refusing to hire a
well-qualified candidate because they were a smoker.

Being told to document a released product to ignore a
known bug and the practical workaround.
