* back-and-forth

Only if it's an adjective, as in "back-and-forth motion" (but "moving back and forth").

* Arguing with your boss -- how to lose graciously

That should be an em dash (see below).

* Our new national energy conservation policy---Keeping Our Employees in the Dark---will be implemented on December 23, 2003.

Right--those should be em dashes (but see below).

I do have a copy of CMS 14 but it's at home holding up a corner of my dining table.

If it were closer to hand, you'd see that (for monospace fonts) it recommends one hyphen for an en dash and two hyphens for an em dash, with no surrounding spaces in either case.

In the rare case that a hyphen and an en dash appear close enough together in a monopsace font that the reader might be confused, you might put spaces around the hyphen being used as an en dash, as in
"pages 9 - 12." Most often, though, you'd write "pages 9-12."
