Hmmmm....there really should be no problem in "losing" the header and
footer. (And frankly, even if it is "lost" the fix is as simple as turning
the Same as previous off and copying the old header/footer into the
incorrect one).

The RD method will work as well, but:

1) You have to be careful about file location. If there is a chance that the
files may need to be moved, make sure that you put them all in the same
directory first and then select the checkbox for keeping the file locations
relative.

2) You have to be careful about numbering across documents. If you are using
numbered chapters, you will have to manually change the chapter numbers for
each document. If you are using consecutively numbered pages in the
document, you will have to manually change the starting page for each new
document.

To use the RD method, click Insert--> Break, select the type of section
break that you want. In the new section, click Insert --> Field. In the
Field Names category, select RD. Type either the file name in the Filename
field and select the Path is relative checkbox, or type the full path name
of the document in the Filename field and leave the Path is relative
checkbox unchecked.

Neither method is perfect....and it depends on what you want to do to decide
what method you prefer.

existing headers and footers during the process), and you should use it
if you absolutely *have* to put all the chapters into a single file.

Do you know, though, that you can easily get a book-wide TOC and index
*without* combining the chapters into a single file?

In the file that contains your front matter, insert a section break and
an
RD field for each additional chapter, something like this:

title page
copyright page
--- page or section break ---
TOC field
--- odd-page section break ---
{ RD chapter01.doc }
--- odd-page section break ---
{ RD chapter02.doc }
--- odd-page section break ---
...
--- odd-page section break ---
{ RD appendixNN.doc }
--- odd-page section break ---
{ INDEX }

This works great for what you want to do.
