Hello Wade,

If your questionnaire is being used by a publisher to
collect information
about yourself that will be used to determine, in
conjunction with a
manuscript, whether or not your book merits
publication then I think that
the question is yes if:

You have either published a book that has an ISBN or
if you have been a
contributing author to such a book and have been
recognized by
acknowledgement within the publication.

If not then the other form of publication is to have a
column in a magazine
or news paper that regularly publishes your content.
The former is however
preferable.
