A clickable index entry is a hyperlink to a page. Even if you do have to use actual bookmarks as the targets, all the hyperlinks to any given page would point to the same bookmark (just as multiple cross-references can point to the same bookmark in Word, and multiple HREF anchor tags can point to the same NAMEd anchor in HTML).

So it seems to me that the number of bookmarks required to hyperlink all the index entries could never exceed the number of pages in the document.
