I usually just ask for a bulleted list; that's not a bad place to start.Then I investigate each bullet point to find out what changes were *really* made.

If you want more, then ask them for the original docs that drove the changes. Ask if they are using any kind of change management system; if so you should be able to extract the requirements for the changes.
