Shlomo Perets has great info on FM & PDF issues at his site, http://www.microtype.com

His utilities and training classes are both excellent.

One side-note: turning on Named Destinations may lead to considerable PDF file bloat; you may want to consider using the Unbloat feature in the Microtype TimeSavers utility.
