A company called Virgina Systems makes Sonar Bookends Activate, which they bill as an "automatic hypertext-link generator for Acrobat." I've used Activate for several years to do exactly what you're trying to achieve: add hyperlinks to the index entries in PDFs created from Word.

The product now includes lots of other options, but that's the only one I've ever used it for, and it does that task wonderfully. It has saved me *far* more than the $199 I paid for it. When questions have arisen, their tech support has been responsive and knowledgeable.

(I have no connection with the company, except as a satisfied user of this one product. I have no experience with any of their other products. I have strong doubts about any product's ability to create useful index *entries* automatically.)
