The CMS has an excellent article on the process. I started by reading this and doing a couple of small indexes manually (index cards and all) so I'd have a good understanding of what and where automation could help. Later, I read several articles and books on indexing, but I like the level of treatment in "The Art of Indexing" by Larry Bonura.
