I don't know whether there is any easily accessible automatic method, but you can certainly manage this with a little bit of manual work:

1. Make separate PDFs of all of the embedded docs.
2. Make a PDF of the main doc.
3. Use the Acrobat Link tool (button that looks like a couple of links of chain) to manually link the icons to the PDFs you made in step 1.

As long as you keep everything in a single directory and choose intuitive labels for the PDF files, you can deliver the whole thing as a single zip file.
