There are two international standards that do just what you asked for:
IS15910 (suitable for use in a contract) and IS18019 (just gives
guidance). You can get them from your organisation's library, or if
you're desperate you can buy them online and download them from

If you don't like what's in these standards, you might also like to look
at which is a project to rewrite both of
