Similar problems have been reported from time to time. This possibility just occurred to me, and it's worth checking out, I think.

I suspect this is related to Word's "feature" of repaginating a document when a new printer is selected. Press Ctrl+P to bring up the Print dialog. See what printer is selected. That's the printer that the bookmarks are based on. When you create a PDF, the printer that's used is Distiller.

Try this. Exit Word. Start > Settings > Printers. Choose Distiller as your default printer. Restart Word and open your document. Check the Print dialog to ensure that Distiller is the selected printer. If not, choose Distiller, close the dialog, and resave the document. Rinse and repeat until you figure out a way to persuade Word that you know what you're doing (may require printing the document once to Distiller or rebooting or who-knows-what). Once you've accomplished that, create a new PDF and see if that solves the problem.
