I had an almost 180 degree different experience where I am now. I was
in under an agency and recently, converted to 1099 with a rate
increase, which means I not only get a gross increase, but I know
keep the money that was going to the agency.

To do this, I received a questionaire with about 15 questions;
payment terms, required notice (for both sides), etc. I answered the
questions and returned it back to legal.

The contract I received to sign about 3 days later was as if my
answers in the questionaire were plugged into slots in the contract

I actually got them to sign off on a 3-week notice with a 40hr/week
buyout for each week that they give me less.
