Thanks for your suggestions. Unfortunately, the PDF Setup doesn't seem to be the problem, or it would be an easy fix. We are including purely headings there. The only style from from the generated LOT file that is part of the Bookmark setup is ListHeading (i.e. "List of Tables").
The only reason we would include the table captions in the PDF setup would be if they could automatically appear under the "List of Tables" bookmark, but as you say, they don't. I haven't seen a way to make the bookmarks do something like that. They are always in the document's order.
...and, we are using FrameMaker, so your advice makes perfect sense to me.

Paragraphs list for a pgf name with an LOT suffix. If, for instance,
your table captions are in TableCaption pgfs, look for the
TableCaptionLOT pgf. Move it to the Don't Include list and click Set.
If for some reason, you want bookmarks for the actual table captions
(this is not usually done, in my experience), then put the actual table
caption pgfs (TableCaption, for example) in the Include list. Note,
however, that the bookmarks are like a TOC, in document order. So those
bookmarks will appear in the bookmark list at the relative positions
where the table captions occur.
