I actually did something like this once myself. It wasn't quite as
extreme, but it did amuse a reviewer. I was supposed to be talking
about making modifications to a screen, and I ended up saying, "Any
medications you make to this screen ... " The reviewer wrote "Take two
aspirins and call me in the morning" in the margin.
My personal favorite has to do with picking the wrong suggestion from a
spell checker.
In the early 90s, a newly minted programmer hadn't checked in his code
for a major testing cycle and I'm sure his manager had a little talk
with him. In the wee hours of the next morning, he checked in the late
code and sent an apology to the whole division for the delay. His
email's closing words: "I'm sorry for any incontinence I may have
caused." Error? Maybe not.
