Macromedia Captivate allows you to do all of those things.

On the start page, under Getting started movies, Additional Features: click
Interactivity.
These features (braching, interactivity and scoring) have been expanded
under Captivate 2.

If you want to go "pro" with your authoring solution look at Adobe
Authorware. keep in mind you will have to cope with the price and the
learning curve.
