I work with XML authoring systems that do what you're describing. We use PTC
/ Arbortext Editor (and related software) for XML authoring and composition
and Documentum for Content Management.

The following threads may be helpful at a minimum by suggesting things you
should be thinking about and careful of:

We need to product PDFs for user guides and CHMs for online help. These are
currently the only two formats we deliver. It could eventually expand into
Web creation if the solution sees more expansion in the company.

PTC's Arbortext Editor will create PDF and CHM and HTML.

Do you have any wisdom to share on what tools or tools packages and
combinations can be used to achieve these goals?
- XML structured authoring (with XML source files that can be
parsed by other custom or third-party tools)

PTC's Arbortext Editor creates native XML. Integration of ANY XML solution
with multiple vendors (even multiple products from a single vendor) may not
be (probably isn't) out of the box behavior. Customizations are required.
There may be specific XML implementations where this is less true (docbook,
DITA), but I'd be suspicious even in that narrow realm that customizations
will be required.

- Content Management

Again, we use Documentum which integrates with XML and Arbortext Editor (and
related products). PTC, Arbortext's parent company, also has a content
management system that is probably worth looking into, especially if you
seriously consider Arbortext products, called Windchill.

- Review tool allowing inputs from SME (Subject matter experts)
without modifying the actual text

We've seen a demo by a company called iMarkup. It did not meet all of our
requirements. In fact, no product has done this yet. If you find anything
promising, please pass it along. This is a business opportunity alert! I
don't know how big the market would be, but "Word-like" review / editing /
commenting capability that preserved the original XML and integrated with
XML authoring tools' change tracking (specficially Arbortext Editor) would
be received with great interest by more than a few.

- PDF and CHM deliverables from the same source.

Again, Editor does this.
