What works for me is the following...
<cut/paste from an internal doc that describes how to update
documentation>

Version is in XXX.YYY format where XXX is upped when it is a content
change and YYY is upped when it is a formatting change.

Include this on the title page and in the footer. I can send you an
example off-list.
