According to the AP Style Guide - The dash, the semicolon, the question mark
and the exclamation point go within the quotation marks when they apply to
the quoted matter only. They go outside when they apply to the whole
sentence.

Thus:

Did I hear him say, "Never again"?

And

Did I hear him say, "Never again?"

Are both valid, depending on whether "Never again?" is the question, or "Did
I hear him say, 'Never again'?" is the question.
