For us, changing a name for an existing product is
an agonizing, 6-12 month process that begins with
distributing change proposals to multiple customers
and government reviewers and obtaining their approvals.
The actual document revs are anticlimactic, and we
can't really "change" the name in the document, but
can only add the new name to a list of the products
the documents are applicable to; the original name
lives on forever. We almost never do this.

For products under development, we are required
to obtain project approval and a project number
prior to the start of development; a "last minute"
name change means at the last minute before a
product development proposal is submitted, because
once the proposal has been submitted, changing
anything in it (name, design goals, whatever) means
cancelling the entre proposal and starting over from
scratch. We almost never do this, either.
