Doctors probably aren't the best comparison here. As I understand it,
doctors aren't employees -- they own their practices, and they partner
with other doctors and/or hire non-doctors to provide services for them.
Even at a clinic or hospital, a doctor is not considered an employee.
Rather, the doctor is the boss. The hospital provides the doctor a
place to practice and associated equipment, staff, and services, and
receives a cut of what the doctor bills.

This is pretty much the opposite of the way it appears to patients, of
course, and juries sometimes look askance at the conceit of "the doctor
is not an employee" when a malpratice award is on the line. But at
least in theory, doctors are more like independent contractor than like
employees, so are neither exempt nor non-exempt.
