Acrobat 9 pro and Word 2003: Links and TOC don't work

We have just upgraded to Acrobat 9 Pro.

When trying to create a PDF from Word 2003 none of the links work, including
the Table of Contents.
In the Acrobat settings, the following option is checked (enabled): "Convert
cross-references and table of contents to links" but it does not work.

I tried to find a solution on Google and searched the Acrobat forum, to no
