When I have worked in regulated industries (DoD, DoE, FAA and FDA so far), it's
been all three.

In unregulated commercial environments, usually high-level overviews and source
management. If the system doesn't automatically preserve incremental versions
with check-in/outs, I upload them manually on a periodic basis.
