This led me to the answer: the comments options that used to be available in the Print dialogue box are now provided (in Acrobat Pro X) through the Comments pane on the right. There is a blue page-like icon on that pane that provides options for creating your comments summary. Play around with it and you'll be able to print only the pages containing comments, with all the comment text expanded and leader lines showing where in the text the comment was entered.

This page shows how to print comments in Adobe 9 and Adobe X. Apparently some of the options in Adobe 9 are not available in Adobe X, one of which is printing "Comments Only". I would suggest going to that help page for more details..

Alas, a big heap o' nothing happens. The Print dialog box returns, unchanged. All the print options are the same.
