Nuance's PDF Converter (or Pro) works fine for converting a PDF to a DOC file while maintaining much of the structure of its source--much better than porting a DOC file from an Acrobat Pro conversion. It's currently at version 7, but all my experiences with it were with version 4, before Nuance bought out its previous developer around five years ago.

There are two or three options, selling for around $50 and $100. Also, Nuance offers frequent discounts for getting it; so look around and wait if you wish to buy it. (Get on Nuance's email list.)
