Ours goes in the front, right inside the cover. Our documents are often
keyed to users by their revisions (Customer 1 is still using Rev A while
other customers have updated to Rev B, etc.), and there are some major
safety issues possible if someone uses the wrong revision.

During my year of doing documents for retail consumer products, the
delivered documents often didn't have any rev histories in them at all.
Just publication dates.
