If the invitation is not personalized, I accept them if I can recall having seen the name before somewhere. I don't need to actually have communicated with the person directly or remember where I've seen his or her name before. Unpersonalized invites from people I've never heard from usually get declined, but I have to say I get very few of those. Mostly from Farmers Insurance recruiters or other MLM-like senders.

I don't send out invitations, except to people I actually know real time or have recently (as in, within the past day or two) communicated directly with. I don't usually personalize them because they'll know who I am. If I think they need a nudge to respond, I'll send them a separate email telling them I've sent an invitation.
