It sounds like the problem wasn't that this writer proofread his or her own material, but that the process, which called for an independent proofreader, wasn't followed. And, I'll bet that doing a self-proof before sending the copy to the official proofreader wouldn't be punished, just replacing the official proofreader with yourself.

Whether that's a fireable offense depends on the circumstances, but if you're in a highly regulated industry or other place where errors have serious consequences, such a firing might be justifiable.
