Check out Adobe Shared Review. File locking isn't an issue because
comments are stored outside of the PDF, in XML files in a folder
created by the "Review Server" (not really a server, per se). When done
reviewing, you can 'flatten' the comments into one PDF (Save As Archive
Copy) for change control history, then dispose of the 'actual' review
PDF and its comments folder.
