The question of whether to use initial caps for all index entries or to use
lower case (except for proper nouns, of course) is a matter of stylistic
preference. Both are "correct" but my own preference is to use lower case. I
think it's easier to scan an index with lower case entries; proper nouns and
other typographical conventions are just a little easier to spot.
