There are many ways this can be done. It's really up to your legal
department to dictate attribution of service marks and trade marks. In lieu
of a legal department, here's one pretty common way to handle them:

- List them all up front on the copyright page (that is, the back side of
the title page). Sometimes I go the company's web site and copy the text
from their legal notices page.
- At the first occurrence in text, mark them with the appropriate marking
(TM, R, SM)
