Horace,

The answer is definitely FrameMaker. It is not very 'sexy' but it does all
it promises.

We also regularly use QuarkXPress (for short design-intensive commercial
docs) and PageMaker (for localization of existing PageMaker documents).
Wouldn't even think of using any of these with anything longer than 20
pages. QuarkXPress doesn't even have automatic TOC and index generation
(you need to purchase add-on Xtensions).

We have never used Ventura, and have never been required to use it.
