I work for the Tech Pubs department at a software development company. We
have a manual titled, _Date Model_, which provides a graphical
representation of the physical data tables used in our system. It is
comprised of several diagrams, keys, fields, and other relational
environment information. We use an application called System Architect to
create this manual. System Architect is a tool that automates the
system-modeling process. You can check out their product at
http://www.popkin.com/prods/sa4.pdf. This is a pdf doc that explains what
they're all about.
