In my opinion, you should not use

What's it really cost?

to mean

What does it really cost?

The correct question would be

What does it really cost?

without the apostrophe. If you insist on using an apostrophe,
reframe the questions as

What's the real cost of it?

I think `What's' is intended as `What is', and not `What does'.
