Since I'm a LONGTIME contractor I'd have to say my questions go something
like this:

Where's the assignment located?
OR a variation: Is the assignment located in Phoenix?
Maybe: Is it on the North side? South? East? West?
This helps you determine approximate driving time.
How soon do they need someone?
What's the rate?
OR a variation: My rate is $xx/hour, will that be a problem?
Note: I always ask for the moon. (Make them gasp!)
You'd be surprised how often I get it!
Will there be overtime?
If OT: What is the overtime rate?
Can we do per diem?
How long is the contract expected to last?

If I've already been submitted on jobs in the area in which the recruiter
indicates his job is located, I usually say, "I've already been submitted on
a job with <company name>. Is this job with <company name>? Usually a good
shop/recruiter will tell you. They don't want to submit you on a job that
you're already submitted on.

When you are submitted for a job, ask the shop for the job requisition
number. This is the number their client company has established for the job,
not the job shop's number. Most give it willingly. If another shop calls you
about the same job, you can tell them you are already submitted, through a
different shop, on Req#xxxxx. Saves everyone a lot of time and trouble.
Double submittals can cause your resume to be thrown out.
