I always ask what software they used to create the documents I'm looking
at, but I haven't asked to see source files. I have, however, encountered
surprises like the one you described, though not quite as bad as that.

At one job, the style guide even said *never* to use FrameMaker's Space
Above and Space Below settings for paragraph leading, but only to add extra
paragraph markers between paragraphs. Because I was helping them move the
documents from Frame 1.3 to 2.0, I also pointed out how using leading
instead of empty paragraphs saved space and made it possible to keep
headings and the following body paragraphs together. Because the manager
who had written the style guide was no longer there, I wrote a proposal and
made the changes myself as I upgraded each document. What a mess it was...
