The AutoShape/callout in Word does work well, if you know how to use it.

1. Use the AutoShapes/callouts command and draw the type of callout you
want.
2. Double-click your callout and format the Fill color to "No Fill".
3. In the Wrapping tab, for Wrapping style select "None".
4. In the Position tab you probably want to make sure you enable "Move
object with text"
5. Set desired preferences in the Text Box tab.
6. In the Text box tab, click the Format Callout button and set your desired
preferences.
7. Click OK to exit from the Format Callout dialog.
8. Now right-click on your callout and select "Set AutoShapes Defaults".
From now on every time you draw that same AutoShape callout, it will have
those attributes.

I agree that setting this up is a bit much, but Word's callout feature works
very well and is HIGHLY customizable.
